Athletic Registration & Handbooks

All Holland Christian Middle School and High School Athletes are required to register online through a program we are partnered with called Final Forms. This online registration is how all students sign up for school sports teams.

For a few co-op teams/non-MHSAA teams, athletes should register on Holland Christian’s Final Forms registration AND will most likely need to register again for another school/organization registration separately (meaning 2 registrations).

Our community benefits from this registration by streamlining our athletic physical process, managing important data, allowing efficient access for coaches and staff, improving communication, and collecting participation fees online.

FinalForms allows you to complete and sign all athletic participation forms and saves your data from season-to-season and year-to-year, meaning you should never have to enter the same information twice!

Athlete Registration

  • Registration includes both a parent account and a student account. The parent account is required to verify registration information each school year (or create a new account for first time parents or athletes) and must select from available sports for their child. Both the parent on the account and the student on the account need to sign all required forms to be eligible.
  • The annual MHSAA physical form is included in the registration. Completing the annual physical is a requirement for eligibility for in-season athletes. The form needs to be printed from the parent account for your MD/DO/PA/NP to perform the required physical. The parent then needs to either scan it or take a quality picture of it with a mobile device and upload the file on the “My Students” page on their account by using their respective child’s document upload button. Holland Christian needs only the 1-page physical form uploaded and NOT all health history information.  If for some reason a parent is unable to upload their child’s physical form, it needs to be dropped off at the child’s respective middle school or high school office. The MHSAA physical must be completed on or after April 15, but before a student can participate in a practice or try-out for an official season during the school year.
  • If you are unable to see your primary care provider (MD/DO/NP/PA) before the start of the next sports season, Holland Christian offers a Physical Night ($25 – Information below: “Sports Physical Night”) and Holland Hospital offers “Walk-In” Care ($45 Information).
  • All parents of middle school and high school athletes will eventually need to register in order to participate, but are highly encouraged to register in April of the preceding year (registration opens April 1st). Registration assists athletes and parents in receiving a variety of communications including summer opportunities specifically for the sports they are registered for.
  • Students are required to have a school issued email account. ZCMS and HC email accounts are accepted. If the student is a transfer student, he or she will be issued their Holland Christian school email account in early August and will need to wait to complete their portion of the registration until they receive this account. The parent account registers students for specific sports and completes the physical form process apart from the student account, but when the HC student email is confirmed, the student can also sign forms and begin receiving communication along with the parent.
  • ZCMS students transitioning from 8th to 9th grade at HCHS will need to change their email to their new HC account when they receive it in August.

Sports Physical Night

Holland Christian encourages every child to see their primary care physician for their well child visit, but also understands the annual MHSAA pre-participation sports physical requirement does not always align with insurance requirements.  Holland Christian’s sports physical night offers a low cost visit that meets the MHSAA requirements, but it is not an alternative to an annual well child visit.  Regardless of where the MHSAA physical is performed, it must be completed by a MD, DO, NP or PA who will check for significant medical concerns that could arise through participation in sports.  The MHSAA physical form must be completed between April 15 and the first practice/try-out for a student to participate in any in-season sport.

New for spring 2024, the Holland Hospital & Shoreline Orthopedic Sports Medicine Collaborative schools are collectively offering two evenings for all partner schools on April 29 at Shoreline Orthopaedic from 6-8pm and May 9 at South Washington Family Medicine from 6-8pm.   Families can register for a physical slot at maroonsathletics.org/Registrations.   Holland Christian has 10 available slots every 15 minutes on both evenings.  

Baseline Testing (Concussion Vital Signs)

Holland Christian High School uses Concussion Vital Signs (CVS) as a tool in the athletic training toolbox to assist healthcare professionals in making decisions about the safe return for athletes to their respective sport after a sustained concussion. Every athlete who participates in a sport at Holland Christian High School must have a CVS baseline concussion test completed prior to the start of the school year, which is then valid for two years. Athletes are baseline tested for their 9th grade year and retested for their 11th grade year. New 10th and 12th grade athletes need a “catch-up” baseline test completed to participate.

Athletes will receive a link through their email in August to complete the online baseline test if they are registered for a sport for the upcoming school year.  Our Athletic Trainer, Karianne Reger, should be contacted with any questions (kreger@hollandchristian.org).

College Athletics

Holland Christian is proud of many alumni who continue participation as student-athletes beyond high school. It is a privilege to find a collegiate opportunity to continue both academics and athletics, but it is also important to understand the realities of competing in college. Statistically, only 1 in 15 high school student athletes compete in collegiate athletics (.5 million NCAA, NAIA & NJCAA athletes/7.5 million NFHS athletes) and only 1 in 50 high school student-athletes receive any form of college athletic scholarship. Although the responsibility for the transition rests on the athlete, our guidance councilors, athletic department and coaches will assist, encourage and promote our athletes.
